Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week, with rotations including one Saturday per month.
• Required Qualifications: Valid license to practice as a physical therapist in the state of California; BLS (Basic Life Support) certification; minimum of 2 years of experience as a physical therapist in an acute, home care, or rehabilitation setting.
• Preferred Qualifications: Good oral and written communication skills; organizational skills; experience working in home health environments.
• Additional Information: Position involves clinical assessments, evaluating home environments, instructing patients and caregivers, supervising home health aides, and participating in peer consultation and ongoing staff competency assessments. The provider will utilize Homecare Homebase (HCHB) charting system. The role is based in Woodland, CA, with service coverage across the surrounding areas.
