Aging Care Manager 2 (Local Government)

The Aging Care Manager 2 position with the Pike County Area Agency on Aging (AAA) offers a chance to make a real difference in the lives of older adults. As an Aging Care Manager 2, you will perform the following duties: Care Planning: Develop informed care plans based on thorough medical, social, psychological, and functional assessments Safety Compliance: Ensure legal and regulatory time frames are met while following all policies, procedures, and statutes Investigative Work: Conduct detailed investigations into situations involving possible abuse, exploitation, abandonment, or self neglect Service Coordination: Complete referrals, arrange needed services, and monitor follow up to maintain service quality Reassessment Tasks: Perform periodic reviews to identify changes in consumer status and adjust support plans accordingly Advisory Support: Provide guidance to staff and the public on legal, medical, and privacy matters affecting protective service consumers Work Schedule and Additional Information Full-time employment Work hours are 7:30 AM to 4:00 PM, Monday - Friday, with 30-minute lunch. There are on call requirements with this position. Telework: You will not have the option to telework in this position. Salary: In some cases, the starting salary may be non-negotiable. QUALIFICATIONS Minimum Experience and Training Requirements: Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
