As a faith based Federally Qualified Health Center (FQHC), we serve approximately 60,000 patients each year across our Memphis health centers, offering primary care, dental, behavioral health, pharmacy, and specialty services to the communities that need them most.About the RoleWe are seeking a clinically sharp, team-oriented Clinical Pharmacist to provide clinical pharmacy services for ambulatory CCHS patients in collaboration with our health center staff providers. In this role, you are an integral part of the PCMH care team - assessing patients, developing and implementing therapeutic plans, writing prescriptions, and partnering closely with physicians, nurses, and behavioral and spiritual health colleagues to keep high-risk patients from falling through the cracks. In this role it means thorough, accurate, and prompt documentation, sound clinical judgment, and pursuing the highest standard of pharmacy care - while staying teachable: welcoming feedback, admitting mistakes early, and continuing your education throughout your career.What You'll DoProvide care for outpatients within your scope of training, in collaboration with CCHS providers - including assessment, ordering and interpretation of data, and development and implementation of a therapeutic plan, including prescription writing and procedures within your expertise, protocols, and available equipment, with appropriate follow-up; document all completed work thoroughly, accurately, and promptly in the practice management system.Partner with your assigned clinic's PCMH team - PCPs, Behavioral Health Consultant, Spiritual Health Provider, nurses, and support staff - to improve patient health, identify high-risk patients in the daily huddle who need detailed PCP review or a separate visit with you, and help maintain the overall cohesion of the team.Complete one-on-one clinical visits with patients assigned to your schedule by Site Lead Physicians or Site Managers.Organize and teach classes assigned by the Chair of Medicine, Chair of Pharmacy, or Chief Clinical Officer.Attend staff and provider meetings and maintain a level of involvement with health center committees and the pharmacy department as needed to ensure quality patient care.Participate in peer review, performance evaluations, improvement data, and other activities to assess and advance quality improvement measures.Meet productivity expectations.Collaborate with CCHS health center physicians and pharmacists to achieve quality care and assist in the educational training of students when asked.Participate in Continuing Education, including Life Support Training, to maintain the skills necessary for your duties and as required by your boards and the state.Sustain the health necessary to perform your duties and maintain current immunization and PPD status.Notify CCHS promptly of any issues regarding drugs, felonies, discipline, loss of license, or other serious matters.Regularly check mail, email, and voicemail, and respond promptly and professionally to all inquiries.Perform other duties as assigned by the Chair of Medicine.What You'll BringDoctor of Pharmacy (PharmD) degree.Post-degree residency, or Board Certification in Ambulatory Care Pharmacy.Current Tennessee pharmacist license and BLS certification.A high level of concentration and attention to detail - this role requires close, intermittent focus, as the number of steps and variables involved creates the possibility of errors without careful attention.Physical readiness for an active clinical role: this position is approximately 80% standing, 30% walking, and 20% sitting, with about 10% of time spent lifting, pulling, or pushing patients and equipment (up to 75-100 lbs., with or without assistance).Comfort using a computer, photocopier/fax/scanner, and medical equipment as part of daily work.Comfort working with patients of all ages and pay ranges, including patients with mental illness, infectious diseases, and exposure to blood and body fluids.What We OfferCompetitive salary and comprehensive benefits, including medical, dental, and vision coverage.Generous Paid Time Off (120 hours in your first year) plus paid holidays.401(k) retirement plan.Annual paid Restorative Day supporting wellness and work-life balance.Mission-driven culture, employee recognition programs, and the opportunity to do work that directly expands healthcare access in Jackson.
