Job Description:

Duties include but are not limited to the following:

• May be required to drive long distances for placement appointments visits for the individuals
• Escorts individuals to appointments such as doctors visits leisure/ recreational activities and discharges as necessary
• Utilizes all appropriate security and safety precautions according to established guidelines
• Provides basic patient care and works with individuals in a mental health setting to develop daily living skills
• Completes required documentation in patient and unit records
• Takes vital signs as needed and documents accordingly in the Electronic Medical Record
• Ensures safety and security of individuals
• Observes and documents patient data behavior and activity
• Prepares individuals for treatments or examinations
• Transports and/or accompanies individuals to treatment units
• May assist with bathing or activities of daily living

Preferred Skills/Qualifications

Preference will be given to candidates who in addition to meeting the minimum qualifications demonstrate some or all of the following skills/experience:

• High School Diploma or GED and a valid Drivers License from the state in which candidate resides
• 1 years direct care experience preferably some experience in Mental Health
• Must have a clear MVR record and meet the Policy requirements for operating a State-owned vehicle: must be 21 years of age and have a minimum of 3 years driving experience.
