Registered Nurse Clinical Manager

Position RN Clinical Manager Location Branch Location: Mayfield, KY This is NOT a remote or work‑from‑home position. You will sit on‑site at our Mayfield, KY branch location. Schedule Work Schedule: Full‑time / 40 Hours Responsibilities Develop, plan, implement, analyze, and organize clinical operations for a specific location. Conduct or delegate assessment and reassessment of patients, update care plans, and interpret patient needs per company, physician or health facility procedures. Manage assignment of caregivers. Oversee delivery of care to all patients served by the location, receive case referrals, review patient information, determine home health or hospice needs, and make admission decisions. Assign appropriate clinicians to a case and guide clinicians to promote performance and quality home‑care services; available during operating hours to assist clinicians. Assist clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing patient Plan of Care (POC). Monitor cases to ensure documentation compliance with regulatory agencies and third‑party payers, and ensure final audits/billing comply with Medicare regulations. Work with the Branch Director and Company Finance Department to establish location revenue and budget goals. Participate in sales and marketing projects. Oversee all clinical employees assigned to a specific location, perform supervisory responsibilities following company procedures, handle corrective action and discipline issues with HR and the Executive Director. Participate in interviewing, hiring, training, evaluating performance of direct‑care clinicians, recommend in‑service education, and ensure adherence to internal policies. Assess staff education needs based on clinical documentation review and conduct regular staff education. Responsible for QA/PI activities; work with Utilization Review staff on data tracking for performance review and outcomes of care analysis. Provide direct patient care infrequently in emergencies. Act as Branch Director in their absence. Participate in performance improvement activities, maintain ongoing clinical knowledge, provide interpretation and direction to staff. Maintain relationships with referral/community sources, participate in professional organizations, and conduct care‑related programs. Qualifications Graduate of an accredited School of Nursing. Current state license as a Registered Nurse. Proof of current CPR. Valid driver’s license, auto insurance and reliable transportation. Minimum two years as a Registered Nurse, with at least one year of management experience in a home care, hospice or equivalent environment. Home health experience required. Management and people leadership experience required. OASIS experience required; OASIS certification (HCS‑O, COQS, and/or COS‑C) preferred. Homecare Homebase (HCHB) experience preferred. CMS PDGM billing knowledge and/or experience preferred. Additional Information Normal Hours of Operation: M‑F / 8a‑5p (CT). On‑Call: As needed. Branch Size: 70 Census (4.5 STAR rating). Annual Bonus: Eligible for the annual incentive bonus with quarterly and annual payouts. Scheduled Weekly Hours: 40. Pay Range: $77,200 - $106,200 per year (starting base pay; may vary based on location and individual qualifications). Job eligible for a bonus incentive plan.
