General Description:

Motivated, results driven individual who will be responsible for all aspects of client management and/or marketing Menorah Park of CNY, Inc. services, within various hospital systems. Individual enhances continuity of patient care between Menorah Park of CNY and referring hospitals and their staff. Coordinates admission clinical and financial information under the discretion of the Administrator and Director of Nursing.

This will include, but is not limited to: pre-admission clinical/financial assessments; establishing relationships with hospital referral sources; and promoting Menorah Park of CNY to hospitals and physician's offices, resulting in developing and maintaining census.

Minimum Training, Experience and Certification Requirements

LPN Certification Preferred

· A graduate of an accredited school of nursing, medical record management, or years of experience in the direct admissions of residents to nursing homes.

· Have experience in geriatric nursing with sensitivity to the total needs of the older adult.

· Have Menorah Park of CNY cell phone in place during business hours (8a/6p) and be flexible and on call for hospital admission needs.

· Ability to operate computer-based data systems and utilize computer to produce all required documentation and reports.

· Familiarity with long-term care and sub-acute facilities preferred.

· Experience communicating to clients, doctors, supervisors, and peers.

Working Conditions

· Works in office area(s) as well as throughout Menorah Park of CNY campus.

· Sits, stands, bends, lifts and moves intermittently during work hours.

· Is subject to frequent interruptions.

· Is involved with residents, personnel, visitors, government agencies personnel, etc., under all conditions and circumstances.

· Is subject to hostile and emotionally upset residents, employees, etc.

· May work beyond normal working hours, on weekends, holidays, and in other positions temporarily in staff emergencies, and to ensure proper care for all residents.

· Must be able to lift, pull, push, and move equipment, supplies, etc.

Education:

• High school or equivalent (Required)

Experience:

• Nursing home: 1 year (Required)
• LTC admissions: 1 year (Preferred)
• Computer skills: 6 years (Required)
• Customer service: 3 years (Required)
• Medical terminology: 4 years (Required)

License/Certification:

• NYS Driver's License (Required)

Willingness to travel:

• 50% (Required)
