Ocupational Therapist

Location: Holton Manor – Elkhorn, WI (on-site position) Status: Full-Time | Part-Time | PRN Flexible Scheduling: Days, PMs, Saturdays – choose what fits your schedule Job Summary This position will provide and administer occupational physical therapy to patients and residents in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. The program is an in-house therapy program, ensuring consistent productivity levels. Key Responsibilities Must uphold professional, legal, and ethical standards of practice for occupational therapy as defined by the AOTA and State Practice Act Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the rehab department Performs initial patient examination, evaluation, interpretation of referrals, and creates the initial plan of care for each patient and revises as appropriate Creates and maintains a patient record for each patient treatment Provides on-site assessment and re-evaluation of each patient’s treatment according to the State or Federal required interval, whichever is more stringent, and adjusts the treatment plan as appropriate Coordinates discharge plan and decisions with the COTA Demonstrates knowledge and appropriate application of precautions and contraindications to occupational therapy treatment procedures and modalities Completes documentation according to facility policies including timeliness and medical necessity support Has primary responsibility for occupational therapy care rendered by the COTA for all patients/clients for whom the therapist is the record Delegates appropriate portions of the treatment plan and program to the COTA consistent with the COTA’s education, training, and experience Must always be accessible by telecommunication to the COTA while the COTA is treating patients for whom the therapist is the record Provides supervision of the COTA as required by State Practice Act Provides supervisory conferences and visits as required by State Practice Act and responds to a COTA’s request for additional supervisory visit for a re-examination Performs other duties as required and/or assigned Qualifications Graduate of an accredited educational program in occupational therapy Licensed in the state in which they will be practicing Must not be excluded or sanctioned from state or federal program Demonstrate professional attitude and appearance Proficient in administrative and communication within department and facility and company Thorough knowledge of the principles, methods, materials, and equipment used in the practice of occupational therapy Display working knowledge of multidisciplinary practice scopes in the Outpatient, ALF, ILF and/or LTC setting Thorough knowledge of all payer source documentation and payment requirements Ability to use analytical skills to develop outcome‑driven solutions Benefits Compensation dependent on experience Health insurance options designed for flexibility Dental and vision insurance Company‑paid life insurance 401(k) with employer match PTO, true flexible scheduling, and shift differentials Tuition reimbursement and continuing education Employee Assistance Program (EAP) Referral bonuses, employee discounts, early wage access Benefits vary by employment status (Full-time, Part-time, PRN) What You Can Expect Hiring manager contact within 1–2 business days 1–2 interviews with leadership and mentor Electronic onboarding and paid orientation Community tour, badge, keys, and ongoing paid training Equal Opportunity Employer All qualified applicants will receive consideration for employment without regard to protected status. #J-18808-Ljbffr Holton Manor
