Registered Nurse Case Manager - Hospice | Pennsylvania | Full-Time Overview This is an opportunity for a compassionate and dedicated Registered Nurse Case Manager to join our client's hospice care team and make a meaningful impact in the lives of patients and families. You will work within a supportive interdisciplinary team, providing essential care coordination, leadership, and patient advocacy across the community. Required Qualifications Education & Experience High School Diploma or GED 1+ year of Hospice experience 1+ year of Management or leadership experience 1+ year of experience in ANY of the following: Home Health Care Epic Home Health Home Health Licensure & Certifications Valid Registered Nurse (RN) license in the state of Pennsylvania (PA) American Heart Association Basic Life Support (AHA BLS) American Heart Association Advanced Cardiac Life Support (AHA ACLS) Additional Requirements Comfortable with local travel across multiple counties Reliable transportation Willing to work required hours; no weekends unless desired Key Responsibilities Manage and oversee patient care in hospice settings Develop and monitor individualized care plans Coordinate care with interdisciplinary healthcare teams Provide leadership, mentorship, and support to nursing staff Ensure compliance with hospice standards, policies, and state regulations Deliver compassionate, patient-centered care to patients and families Territory / Driving Expectations Travel radius: 15-25 miles surrounding the assigned community Daily local travel required across multiple counties Schedule Monday - Friday | 8:00 AM - 6:00 PM No weekends unless desired 2 holidays per year Travel Local travel required throughout multiple counties Mileage or travel reimbursement may apply depending on client policy #J-18808-Ljbffr Purposelink Recruitment
