Hospice Physician Designee

Gentiva is seeking a Hospice Physician Designee to provide compassionate, home-based hospice oversight for patients and families. In this role, you will conduct clinical assessments, certify hospice eligibility, and lead comfort-focused care plans in collaboration with an interdisciplinary team. You’ll manage pain and symptoms, guide goals-of-care discussions, and ensure regulatory-compliant documentation.
