This travel assignment supports evaluation and treatment of functional deficits across inpatient, outpatient, and post-acute care settings. The PT will develop and implement individualized treatment programs, educate patients and families, and collaborate with the care team to advance patient recovery from birth through geriatrics within the home health care continuum.

Responsibilities
- Evaluate patients to identify needs, age-related considerations, strengths, and rehabilitation potential; reassess as needed
- Plan and implement individualized treatment programs based on assessment results, orders, and clinical judgment
- Document all patient information in the medical record and track progress toward goals
- Delegate treatment tasks to PTA and support staff as appropriate; supervise delegated treatment as the licensed PT
- Coordinate care plans with patients, families, and other disciplines; provide education on disease, injury, self-management, and preventive care
- Serve as a clinical instructor or mentor when requested; contribute to clinical education for PT/PTA students
- Ensure patient safety, proper use of equipment, and adherence to confidentiality standards; tailor care to age-specific needs across the life span

Requirements
- Active Michigan PT license (LARA-registered)
- Active American Heart Association BLS certification
- Minimum 1 year of physical therapy experience; acute care experience required
- Demonstrated ability to work in inpatient/outpatient/post-acute care settings
- Strong documentation skills and ability to supervise PTAs and support staff
- Submission requirements: two supervisory references with dates employed
