Key Responsibilities:
- Reconcile medications post-discharge.

Qualifications:
- PharmD with experience in inpatient or ambulatory care.
- Familiarity with TOC protocols and Medicare readmission reduction strategies.
- Empathy-driven, patient-centered care skills.
