Contract | Days | Monday through Friday with 2 evenings until 6:00 PM | Outpatient

Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Design functional treatment programs to support effective return to community activities and progress patients toward discharge within an established length of stay
• Provide supervision, guidance, and direction of discipline-specific therapy services for assigned patients, including delegation as appropriate
• Participate in student programs as assigned, providing a positive learning environment with appropriate supervision and completion of student evaluations
• Adhere to discipline-specific State Practice Act and regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements

Required Qualifications

• Active and valid Physical Therapist license in the state of Georgia (or eligibility to obtain)
• New graduates are welcome to apply
• Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Knowledge of and adherence to discipline-specific professional codes of conduct and state practice acts
• Ability to maintain accurate and timely clinical documentation in compliance with applicable regulations and payer requirements

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Familiarity with functional quality indicators, letters of medical necessity, and insurance authorization processes
• Demonstrated commitment to professional development through continuing education and involvement in professional organizations
