Key Responsibilities\nAs a Licensed Clinical Social Worker at Redesign Counseling Hub, your role encompasses a variety of critical responsibilities:

- Collaborate with a multidisciplinary health care team to develop comprehensive treatment and discharge plans informed by psychosocial assessments and accepted social work interventions. \n
- Establish and maintain working relationships with community resource leaders, representing the organization in meetings and on committees dedicated to addressing the social needs of patients. \n
- Provide counseling and crisis intervention services, including thorough assessments and treatment strategies for emotional and behavioral challenges presented by clients. \n
- Communicate proactively with physicians regarding patient progress, needs, concerns, and any potential or actual issues that arise during treatment. \n
- Ensure the maintenance of accurate, timely, and confidential clinical records and progress notes, adhering to department policy and regulatory standards. \n

Qualifications\nThe successful candidate for this position will meet the following qualifications:

- A Master’s degree in Social Work (MSW), with current licensure as an LCSW. \n

While not required, candidates with the following experiences will be highly regarded:
