• Monday - Friday, 8:30am - 5:30pm, closed for lunch
• 15 patients per day with longer visits for continuity
• Clinic setting
• PACE geriatrics training and experience required
• Geriatric population aged 55 years and up
• Hospital privileges required
• BLS certification required
