Job Details:

• Divide time between Skilled Nursing Facility (SNF) setting and infusion/outpatient cardiology clinic
• Primarily based in the SNF setting with only short travel required between closely located sites
• Lead the establishment of a new infusion clinic serving the central region, providing intravenous diuresis to reduce hospitalizations and improve quality of life
• Provide assessment, follow-up, instruction, and collaboration with community cardiologists
• Implement and adhere to Guideline-Directed Medical Therapy (GDMT) for heart failure patients
• Provide coverage of cardiac testing in addition to infusion clinic responsibilities
• Educate patients and families on lifestyle modifications and GDMT
• Serve as an additional resource to affiliated cardiology practices to address gaps in heart failure population care
• Receive direct support from the Chief of Cardiology to help build a new cardiology consultation program within the SNF setting
• Full-time position with hours split determined by patient population needs
• Call not required
• Autonomy to create your own workflow and schedule
• Future leadership opportunities as the new program grows
• Graduate of an accredited Physician Assistant or Nurse Practitioner program required
• Must be licensed or eligible to be licensed as a Physician Assistant or APRN in Connecticut
• Cardiology experience required
• Geriatric board certification preferred
