RN Case Manager Pediatrics

Job Summary:

The Registered Nurse (RN) Public Health Case Manager is responsible for overseeing comprehensive public health case management focused on well-child checkups, immunizations, infectious disease screening, chronic condition management, and case investigations. The role includes training Community Health Aides and Practitioners (CHA/Ps), supporting public health program design and quality assurance, traveling to villages to provide services and instruction, and delivering education on preventive health topics. Effective communication and collaboration with healthcare providers and diverse communities are essential to improve patient outcomes and program effectiveness. Responsibilities:

• Oversee case management for well-child exams, immunizations, tuberculosis and infectious disease screening, and chronic health condition management.
• Train and support Community Health Aides and Practitioners in administering public health testing, immunizations, examinations, and case investigations.
• Participate in the design, execution, monitoring, and quality assurance of public health programs, identifying areas for improvement and proposing strategies.
• Travel to villages to provide on-site services and instruction to CHA/Ps and clinical staff.
• Provide education on immunizations, chronic care, preventive health, diagnostic and surveillance testing to patients and healthcare workers.
• Assist in Health Aide Training and Continuing Medical Education (CME) events for CHA/Ps and clinical staff.
• Coordinate with healthcare providers and maintain accurate patient records and progress monitoring.
• Stay updated on public health developments and apply knowledge to improve patient care and program outcomes.
• Guide and supervise student Health Aides and Health Aides to develop skills in delivering well-child exams and immunizations.

Required Skills & Certifications:

• Current Registered Nurse (RN) licensure in the State of Alaska.
• Diploma, Associate of Applied Science, or Associate's Degree in Nursing required; Bachelor or Master of Science in Nursing preferred.
• Minimum of 2 years' experience as an RN or in a related public health field.
• Current Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) recommended.
• Proficiency with computer applications including MS Office (Word, Excel, Access) and Cerner electronic health records within 3 months of hire.
• Knowledge and experience in pediatrics required.
• Excellent verbal and written communication skills in English.

Preferred Skills & Certifications:

• Experience teaching and working in cross-cultural settings.
• Fluency in Yugtun language preferred.
• Familiarity with the Community Health Aide Program.

• Frequent travel to remote villages to provide direct services and training.
• Work involves cross-cultural communication and sensitivity.
• Must adhere to health and safety protocols when administering immunizations and conducting examinations.
• Ability to work independently in diverse and sometimes challenging environments.

Scheduling:

• Work schedule may include travel days and variable hours depending on village visits and program needs.
• Shift assignments determined based on public health program requirements and community schedules.
