Unit Care Coordinator (Licensed Practical Nurse/LPN)

Life Care Centers is seeking a compassionate LPN Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you’ll provide direct, patient-first nursing care to seniors in a rehabilitation and long-term care setting. Responsibilities include administering medications, monitoring vital signs, assisting with treatments, and accurately documenting care. You’ll collaborate with an interdisciplinary team to support individualized care plans, educate residents and families, and promote excellent healthcare outcomes in a family-focused environment that encourages professional growth.
