Job Details Job Location: 2600 JOHNSTON ST, LAFAYETTE, LA 70503 Position Type: Full Time Education Level: 2 Year or Associate Degree Travel Percentage: None Job Category: Nursing Position Title: LPN Triage Nurse Reports To RHC Clinical Supervisor Job Description Summary The LPN Triage Nurse processes incoming calls from families, patients, and others. The triage nurse cooperates with physicians and the hospice team of multi‑disciplinary professionals to update the plan of care, follow prescribed medical treatments (including pain management and symptom control), and provide education and supportive care to patients and caregivers. Essential Job Functions / Responsibilities Answer all incoming calls in a manner appropriate to the nature of the call, rapidly identify the caller's needs and take proper action to assure the caller's needs are handled. Order all durable medical equipment (DME) for new admits and current patients. Assist in the implementation of improved work methods and procedures to ensure compliance with federal and state laws and company policies. Build and monitor community and customer perceptions of Hospice of Acadiana, Inc. as a high‑quality provider of services. Prepare, coordinate, and communicate the daily RN Case Manager schedules. Participate in quality assessment performance improvement teams and activities. Collaborate with pharmacies, DME vendors, and other agencies for effective patient management. Facilitate delivery or maintenance of provided medical equipment to meet patient needs. Order necessary testing and imaging. Submit accurate and detailed documentation in real time to promote continuity of care in a timely manner. Attend required meetings and educational sessions. Assist with Volunteer Physician management. Position Qualifications Certification/Licensure/Registration: Valid Louisiana Licensed Practical Nurse (LPN). Education: High School Diploma or equivalent; completion of an approved, accredited practical nursing program. Experience: Minimum three (3) years’ experience as an LPN; hospice experience preferred. Additional Requirements: Valid Louisiana Driver’s License, automobile liability coverage, BLS CPR certification. Job Analysis – Environmental / Physical Demands Environmental Conditions: May be exposed to extremes of heat and cold in all weather conditions; may encounter dust, fumes, or gases; must drive in various weather conditions on roads in varying degrees of repair. Working Conditions: May be exposed to infections or contagious diseases; may have contact with patients under a variety of circumstances; may be near moving equipment; will handle emergency crisis situations; travel required. Physical Demands: Continuous sitting, standing, walking, squatting, bending, kneeling, reaching, twisting, crawling, and climbing; lifting and carrying up to 100+ lbs; pushing and pulling heavy objects; use of feet and hands for repetitive movements.
