Overview Position: Physical Therapist Location: Linden, NJ 07036 Employer: JAG Physical Therapy The Physical Therapist assesses patients’ strengths and weaknesses and develops creative and effective treatment plans. Applies discretion and maintains patient confidentiality while fostering a high-energy, positive environment. Uses training to aid patients in healing, reducing pain, and maximizing functional independence. Staff Physical Therapists examine patients, implement treatment plans, and execute plans of care daily. Responsibilities Collect and record patient care data following the format and guidelines of the patient care service, regulatory agencies, and third-party payers, reflecting an understanding of reimbursement systems and their impact on patient care. Prepare for evaluations in accordance with department policies and standards of practice, including obtaining patient history and knowledge of patient diagnoses. Record daily notes, reassessments/progress reports, discharge plans, and communicate with team members; record daily billing and adhere to productivity guidelines. Select and administer tests and measures; synthesize evaluation results to formulate diagnoses, goals, prognosis, and plan of care. Establish treatment plans based on evaluation results, goals, and best practice; collaborate with the interdisciplinary team, patients, and support systems; modify plans as needed. Identify need for consultations and refer patients to other health care professionals or services; initiate and complete discharge plans. Provide education and training to patients and their support systems; train on ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription, and community reintegration. Develop treatment plans and exercise prescriptions for patient caseload; provide safe, effective interventions compliant with JAG policies. Deliver musculoskeletal interventions, including therapeutic exercise, joint mobilization, and modalities; document notes using EMR systems. Oversee support staff (PTA, ATC, PT aides) as assigned and participate in relevant seminars and community outreach. Attend annual JAG Billing and Coding seminars and promote JAG in the community through lectures and presentations.
