Description

The Southwest Youth and Family Services (SWYFS) Neighborhood Safety & Healing program is a place-based intervention designed to de-escalate conflict and interrupt cycles of violence within the West Seattle area with a focus on High Point and Delridge neighborhoods. Violence Intervention Case Managers operate from a trauma-informed and culturally responsive framework. They will be deployed to provide immediate crisis stabilization, restorative mediation, and outreach. They will follow a formal incident response protocol established with SPD Southwest Precinct to provide rapid intervention following violent incidents to prevent retaliation, offering victims, survivors, their families and other impacted youth and young adults a bridge from acute crisis toward long-term stability and recovery. In addition to immediate crisis response, Violence Intervention Case Managers will provide impacted youth and their families' connections to resources that address the root causes of violence with both one-on-one and group sessions.


Essential Functions

Program Implementation

• Conduct weekly outreach in schools, community centers, courts, and known "hot spots" to identify high-risk youth and families.
• Provide rapid response (within 24-48 hours) to community incidents, including firearm-related violence, to offer immediate support and prevent retaliation.
• Utilize trauma-informed communication to engage "system-wary" individuals and motivate them to enroll in deep, holistic services.
• Develop/implement appropriate goals and objectives using Individual Service Plans (ISP).
• Maintain an active caseload of 15-25 participants, follow-up with them regularly, provide support and resources, and monitor their progress towards ISP goals.
• Meet with participants weekly and conduct formal ISP reviews with participants every 8 weeks.
• Provide counseling referrals to clients following community violence incidents.
• Act as a lead advocate in court hearing, school disciplinary meetings, and social services appointments.
• Lead or co-facilitate evidence-based cohorts with trauma-informed, culturally-responsive groups to support behavioral growth.

Documentation and Performance

• Maintain compliance with documentation with the agency's client database, ensuring all interactions are logged within 24 hours.
• Meet regularly with the Violence Intervention Case Management team to coordinate services provided.
• Attend the monthly City of Seattle meeting
• Attend weekly case consultation and/or team meetings.
• Perform other duties as assigned by the Case Management Program Manager.

Requirements

Minimum Qualifications

• Experience working effectively with diverse populations
• Experience providing referral, advocacy & case management services
• Valid WA State Driver's license, proof of insurance and reliable transportation
• Clear driving record with no more than two moving violations in the last three years
• BA Preferred or equivalent work experience
• Computer skills required
• Must be willing to complete a Background Check and Fingerprinting
• Demonstrated ability to communicate in writing and verbally
• Demonstrated ability providing community outreach
• Demonstrated ability to utilize time management techniques to organize and prioritize
• Ability to be flexible and adapt to programming change
• Work evenings & weekends to provide rapid response services

$28 to $32 per hour
