Our organization is looking for a part-time physician specializing in Family Medicine, Internal Medicine, or Geriatric Medicine to join our team. This role involves providing care at Mercy LIFE Assisi House in Aston, PA, and Mercy LIFE Valley View in Elwyn, both situated in the greater Philadelphia area. The position requires a commitment of 20 hours per week.

Under the guidance of the medical director, the Primary Care Physician (PCP) will deliver essential medical services to participants of the PACE Organization (PO). Responsibilities include:

• Conducting assessments and collaborating with the interdisciplinary team to develop care plans.
• Providing direct medical care to participants and evaluating the effectiveness of care plans.
• Working collaboratively with other healthcare professionals while making independent decisions regarding medical needs.
• Performing necessary medical procedures as required.

Position Overview:

Mercy LIFE Assisi House serves as a retirement convent for the Sisters of St. Francis of Philadelphia, located near the Our Lady of Angels Convent. This facility features a person-centered care approach, fostering a family-like atmosphere within its 125-bed, multi-level care environment, which includes specialized units for Alzheimer’s and dementia care.

Mercy LIFE Valley View offers housing and the PACE program specifically designed for senior adults, including a community of deaf seniors. Candidates should be open to learning American Sign Language.

The role entails working two 8-hour days at Assisi House and one 4-hour day at Valley View, with flexible scheduling during the week. Importantly, there is no requirement for night, weekend, on-call, or holiday coverage.

Key Responsibilities:

• Engage as a member of the interdisciplinary team (IDT) for initial, semi-annual, unscheduled, and annual assessments, reporting any changes in participants' baseline status daily.
• Conduct initial and ongoing assessments of participants, develop care plans, and implement them in alignment with the IDT's overall strategy.
• Refer participants to medical specialists as necessary and manage their care within the nursing home setting, ensuring regular visits and communication with nursing staff.
• Oversee participants' use of medical specialists and inpatient care, maintaining follow-up communication regarding outcomes.
• Review medical outcomes to identify best practices and enhance participant health, ensuring thorough documentation and appropriate diagnostic coding.
• Advise the Medical Director on improving accountability for PO services and resource needs as the program grows.
• Contribute to the development of policies, standards of care, and ongoing evaluation of patient care practices, providing training and guidance to staff on medical and quality assurance matters.
• Participate in the provider on-call rotation as needed.

Minimum Qualifications:

• Doctorate Degree (MD or DO) required.
• Active plenary physician license to practice in the state of the PO is mandatory.
• CDS Certification and DEA Registration are required.
• Current BLS certification is necessary.
• Board Certification in Internal Medicine or Family Practice is required; Board Certification in Geriatrics is preferred.
• Willingness to learn American Sign Language is essential.
• A minimum of one year of experience working with frail or elderly populations is required.
• A valid driver's license, proof of insurance, and access to a personal vehicle are necessary.
• Strong communication, problem-solving, and organizational skills are essential, with the ability to engage effectively with diverse groups.
• Proficiency in Microsoft Office Suite is required.
• Ability to adapt to changing priorities and demonstrate critical thinking skills while managing workload effectively.
• Periodic travel within the Center's geographic area may be necessary.
