General Purpose

The purpose of Transportation Services is to increase or maintain the self-sufficiency of eligible individuals by providing access to human and community services through a coordinated transportation system.

Examples of Duties

Transportation for elderly persons, especially those living in remote rural areas, provide a link to their communities to access needed services and for socialization. Transportation services make it possible to access the senior center, medical services, social services, shopping and recreation. Additionally, vehicles are used for the delivery of home meals. Priority is given to transporting individuals to and from the senior center and the delivery of home delivered meals. Driver is responsible to check the board for cancellations and count the meals going into the van. Participants that are new to the service are to be informed of the service, procedures for obtaining and scheduling a ride and be informed about the service contributions. Annually, this information should be made available to all participants.

Required Qualifications

All service records must be kept confidential and kept on file for five (5) years. 1. Vehicle a. Vehicle Trip Logs are to be maintained on each vehicle and should include, at least, the date of the trip, driver name or identification #, the number of passengers per trip and the mileage. b. Vehicle maintenance and repair reports. c. Daily, pre-trip inspection log. d. Reports of accidents or incidents that related to driver or vehicle safety. e. Documentation of adequate insurance coverage. 2. Driver a. Current Driver's (operator's) License. b. Annual Arizona Department of Transportation check of Driver's record. c. Pre-Service Physical exam and Physical exam. Documentation of employee training. Level One Fingerprint Card 3. Participant a. Participant Registration Card, with emergency information updated every six months or sooner, if needed. Participant signature for receipt of each ride. Should the participant be unable to sign his/her name, the participant must mark an "X" and the driver/escort must sign the participant's name and initial the signature. Individuals providing transportation must be a minimum of 18 years of age and possess a valid Arizona operator's license or an ADOT approved alternate certification. Drivers must be licensed based on the type of vehicle operated. Individuals providing transportation must obtain a pre-service physical exam and have an in-service physical exam every two years thereafter. The provider agency must check the driving record of each individual providing transportation with Arizona DOT, to assure no revocation or suspension of licenses during the past year. Individual providing transportation must obtain a Level One Fingerprint Card. Individuals providing transportation services shall carry a provider agency Identification card. Individuals providing transportation must be provided appropriate training in the use and operation of the vehicle and all vehicular equipment including but not limited to wheelchair and ramps. Additionally, drivers are to have training in: First Aid and CPR Fire Safety Risk Management Working with the Elderly and Disabled Use & abuse of drugs and alcohol in/or affecting the workplace. PASS Certified Program-Passenger and Service and Safety Driver Program every 3 years. Pinal-Gila requirement. Assistance is to be provided to participants in boarding and disembarking the vehicle, assuring that adequate time is allowed to complete each task safely.
