Patient Care Associate (Part-Time) - CHS (OTxHU)

Position overview Under the supervision of the Assistant Director of Nurses, the Patient Care Associate will work in the OTxHU program to assist professional staff with clinical care tasks such as venipuncture, finger‑stick testing, rapid HIV testing, and emergency response. The role includes clerical and care duties that support patient safety, comfort, and well‑being. The Patient Care Associate may also work in jail locations and will support continuity of care for patients with complex health conditions. Duties & responsibilities Administer total care (bed bath, partial or complete). Assist with patient personal hygiene – skin, mouth care, shaving, etc. Transfer patients to other departments, in & out of bed, stretcher and wheelchair; perform physical & mechanical lifting. Position patients and perform range of motion exercises. Assist with ambulating, toileting and all ADL as directed. Prepare patients for meals and assist with feedings as needed. Assist with unit admissions and transfer of patient. Make beds & tidy patient area for bedridden patients. Take and document vital signs (BP, TPR, weight, intake & output) in EMR; perform ECGs and fax abnormal results. Perform dry sterile dressing changes and assist with other treatments. Apply hot & cold therapies under supervision. Maintain intake & output for oral & body excretions. Inform MD/Nurse staff of any changes in patient status. Perform urine dipstick testing and other laboratory documentation. Chaperone staff in all clinical areas when requested. Conduct one‑to‑one observation. Maintain competency in nursing skills as detailed in the performance checklist. Assist with nursing follow‑ups and transmit necessary laboratory information electronically. Act to maintain a safe environment and use resources efficiently. Wait for relief from relief staff at end of shift. Document all encounters in Electronic Medical Records. Complete encounter forms each shift. Perform HIV pre‑ and post‑test counseling and rapid HIV testing. Enter data in rapid HIV database & EMR; fax results each tour. Perform quality controls, ECGs, and nursing follow‑up as assigned. Assist with admission processing, venipunctures, and emergency responses. Clean equipment (thermometers, BP machine, ECG, suction, refrigerators) and replenish supplies. Maintain cleanliness of treatment & exam rooms and secure keys/equipment. Lift up to 100 pounds; stand for extended periods; possess fine motor skills and visual acuity. Minimum qualifications High school diploma or equivalent. Completion of training that includes phlebotomy and ECG (Phlebotomy Technician, EKG Technician, Patient Care, or Medical Assistant programs). Valid and current Basic Life Support (BLS) certification through the American Heart Association. Special note Assignment to a long‑term care facility requires New York State Nurse’s Aide certification.
