Behavioral Health Care Coordinator

Job Title Behavioral Health Care Coordinator Department Behavioral Health Reports To Director of Behavioral Health Status Non-exempt Summary This position uses an integrative approach to promote wellness and address the medical, behavioral health, substance use, and social determinants of health of an assigned panel of patients. Functions as a member of the multidisciplinary treatment team to assess needs, develop, implement and revise comprehensive care plans for children, adults, and families. Coordinates with the care team and other community resources and supports to optimize outcomes. ESSENTIAL DUTIES AND RESPONSIBILITIES Responds in real‑time to meet with patients with needs identified during another PHS appointment, develops a plan to address, and coordinates care. Provides advocacy, linkage, and referral to community resources. Engages patients and families to develop and modify self‑management goals. Promotes health education and encourages patients to establish a healthcare home. Educates and encourages patients to engage in preventive care and establish treatment plan for chronic disease. Coordinates care with community services, agencies, schools, and health care providers. Works effectively as a member of a multidisciplinary team using a unified care plan. Participates in multidisciplinary treatment team meetings and team huddles. Documents patient care including consent, assessment, intervention, and care coordination in a timely manner. Participates in Quality Improvement (QI), Quality Assurance (QA) and Continuous Quality Improvement Activities (CQI) as appropriate. Participates in supervision meetings and other meetings as designated by supervisor. Other duties as assigned. Qualifications Education and/or Experience: Bachelor's degree from a four‑year college or university; 1‑2 years of previous experience in linking consumers with various community resources preferred. Preferred educational preparation in social work or integrated behavioral healthcare. Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization. Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills: Ability to gain knowledge of current practice management system, electronic medical record, Microsoft Word, text paging, Internet, and Intranet. Certificates, Licenses, Registrations: LPN certification. Must hold a current, valid license to practice practical nursing in the state of Ohio. Other Skills, Knowledge and Abilities: Ability to speak Spanish is desirable. Knowledge of examination, diagnostic, and treatment room procedures. Knowledge of medical equipment and instruments to administer patient care. Skilled in taking accurate vital signs. Skilled in maintaining records and recording test results. Skilled with geriatric patients and patients in lower socio‑economic sectors of the community. Physical Demands While performing the duties of this job, the employee is frequently required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is occasionally required to sit and stoop, kneel, crouch, or crawl. The employee must regularly lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. Work Environment While performing the duty of this job, the employee is occasionally exposed to fumes or airborne particles; toxic or caustic chemicals and risk of radiation.
