Overview

The Sub-Acute and Long-Term Care sector is rapidly growing, offering excellent opportunities and a flexible work-life balance (no on-call duties or weekend shifts)! We are looking for a healthcare provider for a part-time role, requiring 32 hours per week, divided between two facilities located in Frederick, MD, and Walkersville, MD: Autumn Lake Healthcare at Ballenger Creek and Autumn Lake Healthcare at Glade Valley. This position is open to either a nurse practitioner or a physician assistant, with a minimum of one year of relevant experience required.

The salary for this role ranges from $56.73 to $59.13 per hour, along with an incentive compensation bonus program that has no cap.

Responsibilities

The Advanced Practice Provider will report to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaborating on-site with the Center Leadership Team to meet specific goals and priorities. The Advanced Practice Provider will also work alongside the Medical Director and/or Attending Physician to deliver patient-specific medical care.

Key responsibilities include:

  • Providing primary and urgent care services to residents in nursing centers.
  • Collaborating with other medical providers to enhance patient care.
  • Working with Center Leadership to fulfill our mission of delivering high-quality healthcare with compassion.
Qualifications

To qualify for this position, the Advanced Practice Provider must meet the following criteria:

  • Graduation from an accredited School of Nursing program and licensure as a registered nurse.
  • Certification as a Nurse Practitioner, having completed an accredited Nurse Practitioner Program with a Master’s Degree in Nursing, focusing on Older Adult and/or Family Nurse Practitioner (FNP). Other specialties may be considered based on experience and organizational needs.
  • For Physician Assistants, graduation from an ARC-PA accredited program and successful completion of the PA National Certifying Exam (PANCE) to obtain the PAC designation is required. State licensure and NCCPA certification are also necessary.

A minimum of two years of experience working with adult or geriatric populations is preferred. In states where practice regulations permit, an APRN must hold an unrestricted professional license and DEA registration for each state of practice. Similarly, PAs must have an unrestricted professional license and DEA registration for their respective states.

Salary Range

The posted salary range for this position is between USD $56.73 and USD $59.13 per hour.
