Home Health Physical Therapist

Santa Clara County & Greater Bay Area | Full-Time / Per Diem | $130,000/year (FT) | $75/hr (Per Diem)

Role Overview:

We are seeking an energetic, geriatric-focused Physical Therapist to partner with senior patients in restoring mobility, function, and independence in the home setting. This role focuses on prevention, rehabilitation, and functional optimization to maximize patient quality of life.

You will work across Santa Clara County and the broader Bay Area, providing in-home physical therapy services as part of a coordinated care team.

Key Responsibilities

• Develop individualized Plans of Care based on patient goals and functional needs
• Provide evidence-based, cost-effective interventions to improve mobility and independence
• Reduce reliance on medications and support non-surgical treatment pathways when appropriate
• Design and implement personalized treatment programs using multiple therapeutic modalities
• Create fitness, balance, strength, and wellness-focused programs tailored to geriatric patients
• Motivate and support patients throughout rehabilitation to maximize functional outcomes
• Promote healthy aging through improved strength, flexibility, coordination, and balance
• Collaborate with physicians, clinicians, and interdisciplinary care teams
• Monitor, evaluate, and communicate patient progress and treatment outcomes
• Accurately document care services in the EMR system in a timely manner (Oasis Kinnser (WellSky) )

Candidate Profile

• Licensed Physical Therapist (California required)
• Graduate degree or higher in Physical Therapy
• Minimum 1 year of experience in Skilled Nursing Facility or Home Health preferred
• Strong geriatric experience highly preferred
• Excellent clinical judgment and evidence-based practice knowledge
• Ability to manage diverse patient personalities and care needs
• Strong interpersonal and communication skills
• Professional, reliable, and patient-centered approach
• Commitment to continuous learning and clinical growth
• Current CPR certification required

Job Conditions

• Full-time onsite home health role (travel required across assigned territory)
• Must be able to work in patient homes throughout Santa Clara County and surrounding areas
• Valid driver’s license and reliable transportation required
• Physical ability to assist patients with mobility and functional tasks required
• Role requires strong documentation and EMR compliance
