In-Home Field Care Manager RN

Humana

Humana is seeking a Field Care Manager RN to assess and guide Medicaid members toward resources for optimal wellness. The role includes developing plans of care, coordinating with stakeholders, and frequent home visits across Illinois as part of a hybrid home/office schedule.

Requires Illinois RN license, 2+ years in-home case management, and comfort with EMR systems. Travel is substantial, with a typical 40-hour week and possible on-site office in Schaumburg, IL.
