Job Details: Schedule: Monday–Friday, 8:00 AM–4:30 PM Location: Easton, MD Coverage Area: Caroline, Talbot, Dorchester & Queen Anne’s Counties Job Type: Travel Contract Contract Length: 13 weeks On-Call/Weekend: No Mileage Reimbursement: Yes – between patient visits Pay Range: $50 to $54/ hr on W2 Pay offered is based on experience, expertise, credentialing, and education. Duties: Perform Home Health admission assessments, physical therapy evaluations, discharge assessments, and routine follow-up visits . Develop and provide appropriate patient-centered therapy services based on clinical assessments. Collaborate with the multidisciplinary care team to coordinate patient care and support positive outcomes. Communicate with patients’ providers regarding clinical needs, progress, and plan of care. Complete required clinical documentation accurately and maintain compliance with home health standards.
