You'll have the opportunity to: Build lasting relationships with participants and their families. Evaluate functional abilities and develop individualized treatment plans. Recommend home modifications and adaptive equipment that improve safety and independence. Collaborate daily with physicians, nurses, physical therapists, social workers, and other disciplines. Help participants maintain meaningful daily activities and improve their quality of life. Play an important role in launching a new PACE program and helping establish best practices. Instead of seeing patients for just a few visits, you'll become part of a team that follows participants over time, allowing you to truly see the impact of your care. Qualifications We're looking for someone who is passionate about helping older adults thrive. Required: Graduate of an accredited entry level program in Physical Therapy Current CPR certification Valid Colorado driver's license and proof of insurance One year working with elderly population, home care or geriatric experience Preferred: Ability to work effectively with culturally, economically, and educationally diverse populations Experience working with interdisciplinary teams An interest in preventative, community-based care Why PACE? PHYSICAL REQUIREMENTS:
