Responsible for:

• Providing direct patient care, including medication administration, wound care, and vital sign monitoring

• Developing and implementing individualized care plans in collaboration with the interdisciplinary team

• Maintaining accurate and detailed records of patient care and medication administration

• Assisting with patient transfers, lifts, and repositioning as needed

• Participating in shift report and bedside report with the healthcare team

• Communicating effectively with patients, families, and healthcare team members

• Maintaining a safe and clean environment, adhering to infection control protocols

• Participating in quality improvement initiatives and training programs
