Community RN Career Direct Care Pathway Placement

The placement for the Community RN Career Direct Care Pathway consists of four distinct levels, determined by the RN's experience, education, and qualifications:

• Level I: Less than 12 months of RN experience.
• Level II: 12 months to less than 3 years of RN experience.
• Level III: 3 years to less than 8 years of RN experience.
• Level IV: 8 years or more of RN experience.

Shift: Nights, 40 hours per week, with a commitment to weekends and holidays!

Location: Main Campus - Emergency Room

Job Profile Summary

This position is centered on delivering both professional and non-professional nursing care services as per physician directives. The role encompasses various Clinical Nurse responsibilities, including:

• Assessing, planning, implementing, and evaluating the care requirements of a specific group of patients.
• Providing nursing care and executing identified clinical support tasks within the scope of practice to fulfill the patient’s care plan.
• Delivering professional nursing care to patients.

This role is primarily an individual contributor position, which may involve overseeing the work of less experienced professionals or managing processes and programs. A significant portion of the role involves the design, implementation, or delivery of processes, programs, and policies, utilizing specialized knowledge typically gained through advanced education.

Job Overview

The RN will independently provide direct nursing care to patients and their families, adhering to established policies, procedures, and practices.

Job DescriptionMinimum Qualifications:

• Valid RN Licensure in Massachusetts.
• Current Basic Life Support (BLS) Certification.
• Department-specific certifications relevant to the type of patient care may be required.
• Experience level based on the assigned category (I, II, III, or IV).

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN).

Duties and Responsibilities:

The following duties and responsibilities are intended to outline the general nature of the work and are not exhaustive. Additional tasks may be assigned as needed:

• Engage in and maintain awareness of performance improvement initiatives.
• Evaluate and integrate evidence-based practices and research into clinical care.
• Conduct and document precise clinical assessments.
• Develop, implement, and assess individualized patient care plans.
• Utilize professional nursing judgment to tailor care plans based on patient assessments.
• Manage patient assignments by delegating tasks effectively according to patient needs and team member skills.
• Create comprehensive educational plans for patients and families, utilizing appropriate resources and documenting per departmental standards.
• Support patient and family rights by facilitating informed decisions and advanced directives.
• Continuously evaluate and adjust care plans in response to changes in patient health status.
• Formulate comprehensive discharge plans, incorporating relevant resources and referrals, including community services.
• Assist in the development of new staff members and support their transition into the unit.
• Guide the professional development of department staff through mentoring.
• Engage in professional growth by identifying learning needs and pursuing relevant education.
• Participate in departmental committees as required.

Physical Requirements:

• Occasional lifting of 30-35 lbs and assisting with patient mobility.
• Prolonged periods of standing and walking.
• Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.
• Exposure to infectious diseases and potential carcinogenic substances.
• Risk of exposure to blood-borne pathogens.
• Fine manual dexterity required for patient handling.
• Visual and auditory acuity necessary for monitoring patient care.

Skills & Abilities:

• Effective communication skills with patients, families, physicians, and the healthcare team.
• Knowledge and application of the nursing process.
