It's a difference you can feel when you walk through the door.Job Description: We are seeking a Part-Time Physical Therapist to deliver high-quality in-house therapy services to adults with a range of diagnoses and disabilities, with a special focus on supporting aging in place. In this role, you will directly influence residents' quality of life by providing person-centered interventions that promote mobility, independence, and overall well-being. Primary Responsibilities:Conducts screenings of new community residents and those referred by nursing to determine the need for interventionEvaluates residents to obtain data necessary for treatment, planning and implementationInterprets and communicates evaluation findings to residents, family, and other team membersRecords treatment times and documentation by following federal and state laws, rules, and regulationsDemonstrates capacity to understand insurance reimbursement and billing while coordinating careProvides treatment in accordance with established standards of practice, department procedures and productivity standards while adapting treatment for individual needsMonitors resident response to intervention, modifying treatment as indicated to attain goals in accordance with physician ordersRecommends and educates residents, family, and community staff in the use of adaptive equipment and assistive devicesConsistently demonstrates sound judgment in the evaluation, planning, implementation, and follow-up of resident therapy programs including excellent communication with community staffMaintains safety standards including safety stands including compliance with infection control, universal precautions and OSHA stands for healthcare professionalsQualifications:Graduate of an accredited therapy program - Physical Therapist (PT)Current state licensureExcellent verbal, written and interpersonal communication skillsResults-oriented with strong critical thinking and problem-solving skillsBenefits:Employee assistance programPTOAdditional Information:All your information will be kept confidential according to EEO guidelines.Location: Medford, OR
