License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: - Two (2) years of experience in a home setting within the last ten (10) years, which may include working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. - Conducting assessments that are similar to UAS such as OASIS. - Experience providing primary care to homebound individuals. - Care management experience for individuals receiving home and community-based services. - Home care experience as a paraprofessional or Licensed Practical Nurse caring for a minimum of two patients per week. Description: Required licensure: Current RN Licensure in Client. Required experience: Minimum of two (2) years of experience in a home setting within the last ten (10) years. Nurses must have recent experience conducting comprehensive assessments of patients. Home care experience includes working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Experience should include conducting assessments similar to UAS, such as OASIS, and providing primary care to homebound individuals. Care management for individuals receiving home and community-based services, including Hospice and Assisted Living, is essential. Home care experience as a paraprofessional or Licensed Practical Nurse caring for at least two patients per week is necessary. Preferred qualifications include experience conducting assessments in UAS-NY in the last 365 days. Experience caring for a geriatric population and managing chronic diseases in the home setting is highly regarded. The role requires the ability to review health status questions and make evaluations based on UAS criteria for enrollment. Candidates should possess knowledge of professional nursing practices and state regulations governing nursing. Excellent organizational, interpersonal, written, and verbal communication skills are essential. Candidates must be able to effectively communicate with multi-generational consumers and demonstrate sensitivity to their concerns. The role may involve performing multiple complex tasks simultaneously in a fast-paced environment. Responsibilities include performing initial in-home evaluations, reviewing consumer medical documentation, and entering evaluation data into electronic systems. It is important to emphasize continuity of care to reduce fragmentation and gaps in treatment plans. Discussion with consumers regarding healthcare options and coordination of care with interdisciplinary team members is also a key responsibility. Candidates should maintain knowledge of community resources and payor requirements related to the target population. Additional tasks may be assigned as needed.
