Physical Therapist Position

CVHCare is looking for a dedicated Physical Therapist to provide care in the assigned county territory, ensuring the highest standards of therapy interventions. The position may require occasional travel outside the designated service area to secure a full caseload and meet patient demand.

Locations: Vacaville, CA; Fairfield, CA; Rio Vista, CA; Suisun City, CA; Dixon, CA; Vallejo, CA; Benicia, CA; Napa, CA; American Canyon, CA; Yountville, CA

Assignment Duration: This is a 13-week contract position.

Schedule: Day shift with possible weekend coverage.

Responsibilities:

• Conduct evaluations, develop treatment plans, and implement therapy interventions.
• Manage OASIS documentation including Recertifications, Routine visits, Evaluations, and Discharges.
• Ensure compliance with therapy productivity standards and expectations.
• Maintain open communication with patients, families, and healthcare providers.
• Adapt to patient needs, including willingness to see Covid+ and Covid PUI patients.

Required Experience / Certifications / Licensure:

• Current California Physical Therapist License.
• BLS Certification.
• Valid Driver's License and Auto Insurance.
• Annual TB test, MMR, Varicella, HepB, TDap (within the last 10 years), and annual Flu shot.
• Completed physical and drug screening.
