Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, develop, and implement individualized treatment plans based on patient needs
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with interdisciplinary team members to support patient care goals
• Maintain compliance with facility-specific credentialing and documentation requirements prior to assignment start

Required Qualifications

• Active Physical Therapist license required; must obtain permanent Wisconsin state license promptly if using a compact license at start
• Experience in an outpatient physical therapy setting
• Ability to meet contract credentialing and quality assurance requirements
• Must have been separated from any prior employment at this facility for at least one year to be eligible

Preferred Experience / Skills

• Prior experience working in a high-volume outpatient therapy environment
• Strong documentation skills and familiarity with electronic medical records
• Ability to adapt quickly to facility-specific workflows and protocols
