Our organization is a physician-led, accountable multi-specialty group practice with over 650 providers. We are excited to present a fantastic opportunity for a Family Medicine Physician (MD/DO) to join a team of 11 Family Medicine specialists in a newly constructed facility exceeding 60,000 square feet. We are open to welcoming residents and are looking for a physician to fill this position. The work schedule is designed as follows: for full-time roles, there are 34 hours of patient contact, with the flexibility to choose between a 4- or 5-day work week. Call responsibilities will be shared among eligible Family Medicine providers, approximately 1:40.

The Caldwell Health Plaza , which opened last fall, is now a dynamic and fully operational state-of-the-art facility. This Health Plaza caters to community members seeking wellness, diagnosis, treatment, and rehabilitation in a convenient location directly off I-84. It offers a wide range of comprehensive healthcare services for families, including:

• Extended Hours Urgent Care
• Family Medicine
• Pediatrics
• Occupational Medicine
• Diabetes Care & Education
• Sleep Medicine
• Orthopedics
• Heart Care
• Physical Therapy
• Occupational Therapy
• Podiatry
• Maternal-Fetal Medicine (MFM)
• Ear, Nose, and Throat (ENT)
• Onsite Lab, X-ray, and Ultrasound

Qualifications:

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree
• Completion of an accredited ACGME/AOA Family Medicine residency program
• Board certification, or the ability to obtain certification within three years of employment
• Eligibility for Idaho medical licensure to practice in all areas of medicine
• Valid registration for controlled substances with the Idaho Boards of Pharmacy & DEA
