Remote Care Nurse II

Delivering high-quality nursing care from a remote setting, the full-time Remote Care Nurse II will conduct patient assessments, develop care plans, and coordinate with multidisciplinary teams to optimize health outcomes using telehealth technologies. Key responsibilities Conduct thorough remote patient assessments via phone or video conferencing to evaluate health status and identify care needs Develop, implement, and update individualized care plans in collaboration with patients and healthcare teams Monitor patient progress and adherence to treatment plans, providing education and guidance to promote self-care and disease management Required qualifications Current and unrestricted Registered Nurse (RN) license in the United States Minimum of two years of clinical nursing experience, preferably in case management, chronic disease management, or telehealth Proficiency with electronic health record (EHR) systems and telehealth technology platforms Ability to work independently in a remote environment while managing multiple patient cases efficiently License in a compact state with preference for additional licenses in California and Illinois
