Key Responsibilities of Physical Therapy Assistant

• Provide comprehensive physical therapy treatment based off POC and deliver evidence-based interventions to maximize functional independence and quality of life.
• Demonstrate knowledge of the Patient-Driven Payment Model (PDPM), Medicare guidelines, skilled therapy requirements, and appropriate documentation practices.
• Develop and support long-term care programming, including fall prevention, mobility enhancement, functional maintenance, restorative nursing collaboration, and contracture prevention.
• Collaborate with physical therapist, occupational therapy, speech-language pathology, nursing, physicians, and other interdisciplinary team members to coordinate care and achieve optimal resident outcomes.
• Maintain accurate, timely, and clinically meaningful documentation supporting medical necessity, skilled interventions, patient progress, and functional outcomes.
• Participate in interdisciplinary meetings, discharge planning, care conferences, and quality improvement initiatives.
• Monitor patient progress, adjust treatment plans in collaboration with Physical Therapist as appropriate, and promote measurable clinical and functional outcomes.
• Communicate effectively with residents, families, caregivers, and facility leadership.

Qualifications of Physical Therapy Assistant

• Current Florida Physical Therapy Assistant license in good standing.
• Graduate of an accredited Physical Therapy Assistant program.
• Experience in a skilled nursing facility, post-acute rehabilitation, or long-term care setting preferred.
• Strong understanding of PDPM, Medicare Part A and Part B, and skilled therapy documentation requirements.
• Experience with Net Health and Point Click Care preferred.
• Knowledge of long-term care programming, fall prevention, restorative nursing, and functional maintenance programs.
• Excellent communication, organizational, time-management, and interdisciplinary teamwork skills.
• Commitment to clinical excellence, regulatory compliance, and positive patient outcomes.
