Rehabilitation RN Case Manager

Reston Hospital Center seeks an RN Case Manager for our Rehab Unit. In this role, you’ll coordinate care for adult rehab patients, managing assessments, care plans, and safe discharges. You will collaborate closely with physicians, therapists, social workers, and families to optimize recovery, reduce readmissions, and support patient goals. Responsibilities include utilization review, documentation, patient and family education, and coordination of community resources.
