Case Management Coordinator

Job Overview Randstad Healthcare is hiring for a Medical Case Manager in the Miami - Dade area. Candidate must reside in Miami-Dade County and be located in one of the following ZIP codes: 33174, 33144, 33134, 33165, 33175, 33184, 33182, 33172, 33126. Travel of 50‑75% within Miami-Dade County for face‑to‑face visits with members is required. You will see members at their homes, assisted living facilities, and skilled nursing facilities. Responsibilities As a Case Management Coordinator, you will telephonically and/or face‑to‑face assess, plan, implement, and coordinate all case‑management activities with members to evaluate their medical needs and facilitate overall wellness. You will manage a caseload that includes supportive and medically complex members, developing proactive plans to address issues and enhance short‑ and long‑term outcomes. Key activities include: Prior authorizations Coordination with PCP and skilled providers Condition management information Medication review Community resources and supports Qualifications Bilingual in Spanish and English (fluent speaking and writing) Ability to multitask, prioritize, and adapt to a fast‑paced, changing environment Effective communication skills, both verbal and written Self‑motivated, energetic, detail‑oriented, highly organized, and tech‑savvy Experience or willingness to manage caseloads with supportive and medically complex members Must reside in Miami-Dade County Benefits and Compensation Pay will be based on education, work experience, location, duties, certifications, and other factors.
