Position: RN Home Health

Shift: Day 5x8-Hour (08:00 - 17:00)

Job Description

The RN Home Health position involves coordinating and directing the provision of nursing care in patients' homes based on physician orders and following agency policies and procedures. The RN will utilize the nursing process effectively while ensuring high-quality patient care.

Key Responsibilities:

• Provide home nursing care to patients according to physician orders.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Ensure all documentation is completed within 24 hours of patient visits.
• Participate in periodic office meetings and effectively communicate with the clinical manager.
• Handle transportation logistics for home visits; must have reliable transportation and valid driver's license.

Travel Requirements:

The RN will be required to travel for home visits within a designated territory, covering up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate.

Patient Care Details:

• Average number of patients per day: 5-10.
• Expected weekly productivity: 30 points, with each type of visit and activity having a different point value.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.

Qualifications:

• Minimum of 1 full year of Adult Home Health experience or Hospice experience, as applicable.
• Current certification in CPR and Basic Life Support (BLS).
• Proficiency in HomeCare HomeBase (HCHB) and Oasis within the last 6 months is required; retraining may be needed for those outside this timeframe.
• Ability to work the required weekend and on-call rotations as needed.

Work Schedule:

• Monday to Friday, from 8:00 AM to 5:00 PM.
• Weekend rotation every fourth weekend and on-call responsibilities every fourth week.

Location:

Territory covered includes Hull, Halifax, Kingston, Plymouth, Pembroke, Marion, Mattapoisett, and Wareham, MA.

Additional Information:

There are no guaranteed hours for this position. It is essential that the RN be willing to accept all patients scheduled, regardless of appointment distances. Candidates must understand all documentation must be submitted timely to ensure effective patient care and billing processes.
