Registered Nurse Case Manager

Job Summary:

The Registered Nurse - Case Manager is responsible for providing skilled nursing care and case management within an acute hospital setting. This role involves patient assessments, medication administration, treatment planning, and collaborating with interdisciplinary teams to ensure high-quality patient care. The nurse will work primarily day shifts in a 4x10 schedule, supporting acute care patients while utilizing Epic electronic medical records. Strong communication and clinical skills, along with at least one year of acute care experience, are essential. Responsibilities:

• Perform comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed.
• Collaborate with interdisciplinary team members to coordinate patient care.
• Document patient information accurately using Epic electronic health records.
• Provide case management services to support patient transitions and discharge planning.
• Support facility needs by floating within nursing scope as required.
• Adhere to hospital policies and maintain patient safety standards.
• Work weekends on a rotating schedule, approximately every other weekend or 4 weekend days in a 4-week period.

Required Skills & Certifications:

• Active Alaska Registered Nurse (RN) license in hand at time of hire.
• Basic Life Support (BLS) certification.
• Minimum of 1 year acute care nursing experience.
• Proficiency with Epic electronic medical record system.
• Strong clinical assessment, medication administration, and treatment planning skills.
• Excellent communication and interdisciplinary collaboration abilities.

Preferred Skills & Certifications:

• Accredited Case Manager (ACM) certification.

Special Considerations:

• No on-call requirements.
• Must be willing to float within nursing scope to meet facility needs.
• Candidates must reside at least 50 miles from the facility; local candidates are not accepted.
• Limited request time off (RTO) policy: one 5-day or less request per 13-week assignment and no holiday RTO for auto offer.
• Facility is a Magnet designated, Level II Trauma acute hospital with 407 staffed beds.
• Free parking provided; scrub color supplied.

Scheduling:

• Work schedule is 4 days per week, 10-hour day shifts (4x10).
• Exact shift times may vary within the day shift range.
• Weekend work required approximately every other weekend or 4 weekend days in a 4-week schedule.
• Guaranteed 70 hours per 2-week pay period; may be called off for one 10-hour shift per 2 weeks.
