Full-Time Hospice RN Case Manager Impactful Patient Care

HARBOR HEALTHCARE SYSTEM

Harbor Healthcare System is seeking a qualified Registered Nurse (RN) Case Manager for a full-time position in Alexandria, Louisiana. This out-patient role involves contributing to the interdisciplinary care team by coordinating care and managing patient admissions, assessments, and discharges. Ideal candidates will have a valid RN license in Louisiana and at least 1 year of experience. Benefits include healthcare, PTO, and holiday pay, with work hours from 8:00 a.m. – 5:00 p.m., Monday to Friday. #J-18808-Ljbffr
