We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnoses in the most functional setting there is - a person's own home and community. Patients are accepted anywhere on the continuum from severe brain injury to job readiness. We are looking for experienced RN’s who want to provide truly patient-centered care in San Antonio, TX and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes. Areas of responsibility include but are not limited to: Visit patients in their homes for one-on-one visits Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review Administer medication as prescribed by the patient’s physician Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life Provide effective communication to patients/clients, their family members, team members, and other health care professionals Qualifications Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing Current state license as an RN Proof of current CPR certification Compassionate, friendly demeanor Excellent communication skills Current driver’s license and reliable transportation Ability to work a flexible schedule and travel locally Collage Rehabilitation Partners is an equal employment opportunity employer.
