Physical Therapist, PtAdoration Home Health is seeking a Physical Therapist, PT, for a full-time opportunity performing home health visits for our office located in Hagerstown, MD. Our office services adult and geriatric patients on an hourly basis in territories throughout the Washington County area. Provide quality 1:1 care that's individualized and affords patients the opportunity to receive the care required to remain at home.Prior clinical experience as a licensed Physical Therapist is preferred; not required. Come join us and make a meaningful impact with your career today!ResponsibilitiesPerforms physical therapy evaluation and treatment of patients based upon physician orders/referralsCompletes a comprehensive evaluation using appropriate evaluation and assessment tools and proceduresReviews patient records for current diagnosis, past medical history, precautions and contraindicationsEstablishes measurable goals and develops treatment plan to move the patient toward his or her maximum level of independence and/or functionRegularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatmentCommunicates patient’s needs and progress to the treatment team, physician, person receiving services, family members, as applicableConsults with medical providers regarding patient evaluation and treatment, progress of the patient and discharge from treatmentRecommends and assists in securing special adaptive equipment for patient to improve function and incorporates the use of adaptive equipment to enhance the rehab programMaintains and submits evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulationsAdheres to standards of the Company’s Code of Business Conduct, policies, Health Insurance Portability Accountability Act and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal lawsSupervises Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice actAttends, participates in and/or conducts internal staff development programs and meetings; obtains continuing education as required by company policy and regulations; maintains and enhances clinical practice skillsQualificationsGraduate of a Physical Therapist Education program approved by CAPTECurrent Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hireTwo (2) years of experience in adult physical therapy, home health experience preferredExcellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklistAbout Our Line of BusinessAdoration Home Health and Hospice, an affiliate of BrightSpring Health Services, provides quality and compassionate services in the comfort of home, providing support for patients, families, and caregivers in their time of need.
