Job Title: Social Worker Facility: Green Hills Center for Rehabilitation and Healing Join Our Team as a Social Worker! Green Hills Center for Rehabilitation and Healing is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. In this role, you will play a vital part in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and enhancing overall quality of life within our skilled nursing environment.
