About This Role The Registered Nurse in the Ambulatory Clinic will perform an age‑specific plan of care for a designated group of patients, utilizing the nursing process to assess, diagnose, plan, implement, and evaluate patient care. This role is ideal for those who excel in collaboration with healthcare teams and possess strong clinical judgment. Highlights Location: Riverhead, NY Schedule: Full-Time What You’ll Do Assess the patient’s physiologic health status. Conduct patient interviews and explain policies and procedures. Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status. Determine mobility, sensory deficits, prostheses use, and skin condition. Assess level of pain and pain management. Communicate and document patient’s physiologic health status and plan of care. Elicit perception of medical/nursing care and expectations. Develop criteria for measurement of patient outcomes. Formulate the plan of care and establish priorities. Implement the plan of care and maintain patient safety. Requirements Education: Associate’s Degree in Nursing required, Bachelor’s Degree in Nursing preferred. Experience: Minimum 1-2 years of professional nursing experience in the related specialty. Licenses & Certifications: Current license to practice as a Registered Professional Nurse in New York State required. BLS certification required, plus specialized certifications as needed.
