Certified Nurse Assistant | BeLoved Hospice Job Summary Provides direct palliative care to terminally ill patient and their families while functioning within a medically approved plan of care. The Certified Nurse Assistant will deliver services under the direction of the interdisciplinary team and be supervised by a registered nurse. Accountability This position reports directly to the Director of Nursing Works under the supervision of an RN Follows the Plan of Care set out by Interdisciplinary Team Job Duties and Responsibilities Follows and individualized Plan of Care established by the interdisciplinary team for each patient Successfully demonstrates the ability to provide the following care(s) to patients: Bathing (shower, partial, full bed bath) Oral Care (for an alert patient, an unresponsive patient and a patient with dentures) Shaving (with disposable and electric razor) Vital Signs (blood pressure, pulse, temperature and respirations) Basic Nail Care Shampoo Patient’s Hair (in shower and in bed) Use Appropriate Dressing Techniques (for an ambulatory, wheelchair bound, bedbound and unresponsive patients) Provide Effective Skin Care Knowledge of Proper Toileting Techniques Knowledge with Proper Colostomy Care Knowledge with Urinary Bag Devices Appropriate Positioning and Body Alignment Range of Motion Activities Safe Transfer Techniques Reporting Change in Patient Condition to RN Case Manager Notify RN Case Manager when Patient Complains of Pain Basic Homemaker Services Light Housekeeping Services Laundering of Personal Clothing and Linens Meal Preparation/Feeding Companionship Gathers data as assigned by the RN Immediately reports any changes to the RN Case Manager or Director of Nursing Reports unusual conditions, observations and pertinent information to the RN Case Manager. Understands/provides basic principles of palliative care/pain management within the Scope of Practice. Provides personal care as assigned by the RN, which may consist of but not limited to: Certified Nurse Assistant duties, companionship, light housekeeping, etc. Comply with all documentation policies/procedures, which include documentation at bedside. Assumes responsibility for on‑going personal development and continuing education. Creates opportunities for new referrals and referral relationships when representing the company in the community.
