LPN – Long-Term Care

Part-time Day and Evening shifts, scheduled as needed by the facility (generally 2x8s/3x8s). Weekend and holiday shifts required.

Provide direct and indirect skilled nursing care to assigned residents, following defined policies and procedures under the supervision of a Registered Nurse.

Collect and communicate data relevant to residents' physical and psychosocial condition on an ongoing basis to the RN and Interdisciplinary Team.

Contribute to the creation of developmentally appropriate and culturally relevant care plans.

Recommend revisions to care plans, evaluate resident outcomes, and ensure comprehensive care.

Participate in mentoring and orientation of new staff while maintaining current knowledge in Nursing Practice.

Lead in professional practice settings under the direction of the RN.

Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
