Field Outreach Navigator — Community Health Connections
Pacific Health Group
Pacific Health Group is seeking a Field-Based Street Outreach Advisor to connect individuals with care and support services in high-need communities. The role involves in-person outreach, building partnerships, and managing referrals to Enhanced Care Management, Community Supports, and Behavioral Health services. You will spend considerable time in the field, traveling within the county, documenting activities in CRM, and collaborating with internal care teams to ensure referrals are tracked and #J-18808-Ljbffr Pacific Health Group
